Immediate remediation (first 0–24 hrs)

• Contain: activate kill-switch to disable the feature or route traffic to a safe fallback (cached neutral responses) and disable any external integrations.
• Short-term patch: apply urgent prompt-sanitization and output filters (blocklist + pattern-based heuristics) and raise model temperature to reduce hallucinations.
• Monitoring: enable enhanced logging (inputs, prompts, model logits, decisions) and realtime alerting for repeats of harmful categories; preserve all telemetry for forensics.
• Customer support: triage affected users, offer apologies, refunds/credits if appropriate, and prioritize those at risk with escalation.

Communication (transparent, timely)

• To users (within 24 hrs): concise public notice acknowledging the issue, steps taken (service paused/mitigations applied), expected timeline for fixes, and contact channel for sensitive cases. Use empathetic language and commit to a postmortem.
• To leadership & legal: immediate incident brief (impact, scope, risk, mitigation actions), daily updates, and proposed timeline for RCA and remediation. Include PR and regulatory risk assessment.
• To internal teams: incident channel with role assignments, runbook link, and meeting cadence.

Root-cause analysis (48–72 hrs)

• Data collection: aggregate logs, model inputs/outputs, user sessions, prompt templates, and recent model/config changes.
• Hypothesis-driven tests: reproduce failure cases, run adversarial/edge-case sweeps, compare model versions and prompt-engineering changes.
• Analyze training/fine-tune data for bias sources, labeler instructions, and RLHF reward model behavior. Check inference-time components (safety filters, tokenizers, decoding).
• Outcome: map direct cause(s) (e.g., training data skew, reward misspecification, filter gap) and contributing process gaps.

Model governance & technical changes

• Short-term: harden output filters, add explicit safety tokens, implement contrastive safety scoring, and rollback to last safe model if needed.
• Mid-term: periodic bias audits, adversarial red-team exercises, automated differential testing across demographic slices, and stricter CI for model/version rollouts (canary + shadow testing).
• Data & labeling: enforce provenance requirements, diverse labeler panels, updated label guidelines, and automated dataset-bias metrics.
• Policy: mandatory risk assessment for releases, safety sign-off from cross-functional committee (legal, ethics, PM, ops), and SLAs for incident response.
• Observability: retain longer telemetry retention, add counterfactual testing and synthetic scenario suites.

Personal ownership

• I would lead the incident response team until stable, run the RCA, present findings and timeline to leadership, and own the roadmap items for safety fixes. I’d commit to measurable KPIs (zero recurrence for identified failure modes, improved safety-score by X% within 3 months), ensure training for engineers on bias-aware development, and publish an honest public postmortem with lessons and timelines. I’d prioritize rebuilding trust through transparent actions and continuous improvement.

Situation: Our team had standardized on full-model fine-tuning for all downstream NLP tasks—fine-tuning all weights of large pretrained transformers. It was familiar and had delivered solid results. Then several papers and internal experiments suggested parameter-efficient methods (LoRA / adapters) matched accuracy while cutting GPU time and storage by large factors.

Task: I needed to decide whether to abandon our default full fine-tuning, convince stakeholders, and safely migrate production pipelines without regressions.

Action:
