Situation: A product manager disputed a retention metric showing a drop.

Action: I listened fully as they described a recent promo and a change in login flow, asked targeted questions about cohorts and timing, and paraphrased to confirm. That revealed they were comparing different cohort definitions.

5. Remediation Plan & Quick Fixes (within 7 days)

• Action: Prioritize fixes: correct ETL, patch dashboard calc, retract or annotate prior decisions, notify stakeholders.
• Artifact: Remediation backlog with tasks, owners, deadlines; communication draft for impacted stakeholders.
• Owner: Data Engineer (fixes), Report Author (dashboard patches), BI Lead (communications).

6. Preventative Changes (14–30 days)

• Action: Implement process and tooling changes: unit tests on metrics, automated lineage checks, versioned report deployments, mandatory data definitions in catalog, approval gates for critical metrics, SLA on alerting for data schema changes.
• Artifact: Updated BI runbook, metric catalog entries, CI pipeline jobs (tests), RBAC changes, training materials.
• Owner: BI Manager (policy), Data Platform Lead (tooling), Report Authors (definitions).

7. Follow-up Review & Closure (30–45 days)

• Action: Present postmortem to stakeholders, confirm fixes, measure if similar incidents recur, update KPIs for BI reliability.
• Artifact: Final postmortem presentation, closure sign-off, lessons-learned log.
• Owner: BI Lead (presenter), Exec sponsor (sign-off).

Key governance & metrics:

• SLA: initial triage ≤48 hrs, RCA draft ≤7 days, remediation actions tracked to closure.
• Metrics: mean time to detect, mean time to remediate, number of critical metrics with automated tests, stakeholder satisfaction.

Trade-offs & notes:

• Balance speed vs. forensic depth: preserve logs before running destructive checks.
• Where automated lineage tools aren’t available, allocate time for manual verification.
• Ensure blameless framing to encourage honest reporting and adoption of process.
