After 15 moderated interviews, I'd run a 90 to 120 minute affinity-mapping workshop: interviewers pre-distill their sessions into roughly three to five insight stickies each, a cross-functional group of six to eight people clusters those notes into a handful of named themes, and each theme gets converted into a problem statement and a testable design hypothesis before the session ends.

• Timebox and roles. 10 minutes kickoff (goals, persona reminders), 60-80 minutes clustering and naming, 20-30 minutes turning clusters into problem statements and next steps. Roles: a facilitator (usually me) who keeps time and drives decisions; note owners, the researchers who ran the interviews, who read out highlights and place stickies; a decider, usually the PM, who has final say on what gets prioritized; and a scribe who documents decisions and open questions so nothing is lost after the room clears.
• From notes to themes. Interviewers distill each interview into a handful of one-idea-per-sticky observations ahead of time. In the room, everyone silently places their stickies, then the group silently clusters similar notes before anyone discusses them out loud, specifically to stop one voice from framing the pattern before quieter participants weigh in.
• Naming clusters. A good cluster name states the user, the difficulty, and, where possible, the evidence behind it, for example "New users can't find critical onboarding tasks" rather than the generic "Discovery."
• From cluster to problem statement to hypothesis. A problem statement names the user, the behavior, and the consequence. A design hypothesis (still qualitative, not a statistical claim) says what change you'd try and how you'd check it worked, for example with a small follow-up usability test rather than a significance test.

Worked example

Fifteen interviews, roughly four insight stickies each, produce about 60 raw notes (15 x 4 = 60). Clustering converges to six themes. The largest cluster, "can't find required onboarding tasks," pulls together 18 of the 60 notes and shows up in 11 of the 15 interviews, which is about 73% of participants (11/15 = 0.733). That becomes the problem statement: "New users can't locate the required onboarding tasks among the optional prompts cluttering the dashboard, so many stall before finishing setup, a pattern mentioned by 11 of 15 participants." The hypothesis: "If we surface the required tasks as a single ordered checklist at the top of the dashboard, new users should notice and start them more easily; we'll check this with a follow-up moderated test on five or six new users next sprint," rather than promising a specific completion-rate number the qualitative data can't support on its own.

Trade-offs and pitfalls

• Ninety minutes goes fast once naming starts: teams often rush the last step and settle for topic labels ("onboarding") instead of real problem statements, which makes the workshop's output hard to act on later.
• Watch for the gap between "a problem statement" and "a solution in disguise." A statement like "we need a checklist" jumps straight to a fix; keep the problem statement about the user and the difficulty, and put the proposed fix in the hypothesis, where it belongs.
• Always attach the evidence (the count and the source interviews) to each cluster name, not just to a supporting appendix, so the PM in the room can trace any prioritization decision back to specific participants later.

Direct answer

Thematic analysis moves through six steps: get familiar with the data, generate initial codes, search for candidate themes, review those themes against the data, define and name the final themes, then write up the findings. Each step should leave behind a concrete artifact, not just a mental impression, so the product team can trust and reuse the output.

1. Familiarization. Read the transcripts, listen to recordings, jot margin notes. Deliverable: a one-page "data snapshot" summarizing dataset size, a few striking quotes, and early surprises for the product team.
2. Generating initial codes. Systematically label meaningful segments, both surface-level (semantic) codes and codes that capture an underlying idea (latent) codes. Deliverable: a codebook: code name, plain definition, and an example quote, so a designer or engineer reading it later understands exactly what each code means.
3. Searching for themes. Group related codes into candidate themes that describe a pattern, not just a repeated word. Deliverable: a visual cluster map (you can borrow the silent-clustering technique from affinity mapping here) exported as an image showing the emerging structure.
4. Reviewing themes. Check each candidate theme against the coded data and the full transcripts; merge overlapping ones, split ones that are really two ideas, and drop ones that don't hold up. Deliverable: a short validity memo flagging any theme that's still weak, with a note on what evidence would strengthen it.
5. Defining and naming themes. Write a tight description of what each theme actually means and how it relates to the others. Deliverable: a theme card: title, a two to three sentence definition, two exemplar quotes, and the likely business impact.
6. Producing the report. Translate themes into insights and prioritized recommendations. Deliverable: a short deck built as insight, then implication, then a proposed next step, with a raw-quote appendix so skeptical readers can check the sourcing themselves.

Worked example

Ten transcripts about a mobile banking app's transfer feature. Familiarization surfaces a recurring frustration around confirming a transfer. Coding produces codes like "unsure if the transfer went through" and "double-checks the amount before confirming." Clustering those codes (and two others like "wants a receipt immediately") reveals they all point to the same underlying worry rather than three separate complaints, so they become one theme: "Users don't trust that the transfer executed correctly." That's a theme, not just a repeated code, because it names a shared underlying concern that several different behaviors and quotes independently point to, and it's stated as something the team could design against, not just a topic label.

Trade-offs and pitfalls

• The most common mistake is stopping at step 3 and calling a cluster of codes a "theme" when it's really just a frequently occurring topic, for example "users mentioned dark mode." A real theme explains why that topic keeps coming up in terms of the research question, for example "users want more control over how much the app demands their attention," which dark mode requests, notification complaints, and settings-menu confusion might all feed into.
• Skipping the review step (4) to save time is tempting under deadline pressure, but it's exactly the step that catches a theme that only looked solid because of one loud participant.
• A codebook without an example quote for each code is hard for a second coder, or a stakeholder auditing the work later, to apply consistently; always include one.

What "rigorous and unaltered" actually requires you to have kept

Legal isn't asking whether your finding was right, they're asking whether you can show your process and prove nothing was quietly changed after the fact. That means having, from the start of the study:

• A versioned protocol and codebook: the research plan and code definitions you used, with a simple changelog (date, what changed, who approved it), so you can show the codebook you used originally is the same one you're pointing to now, not a version edited afterward to fit the conclusion.
• Dated raw source material: transcripts, recordings, and field notes, saved with a capture date and never edited in place. If a transcript needs correction, save the correction as a new, dated file rather than overwriting the original.
• A coding and memo trail: who coded which session, when, and any memos explaining a judgment call ("coded this as confused rather than frustrated because..."), stored alongside the coded data rather than only living in someone's head.
• A redaction log, separate from the redaction itself: if you removed or altered anything for privacy, keep a record of what was removed, by whom, and why, with the original preserved, access-restricted, rather than destroyed.
• A record of who signed off on the final finding, and when, distinct from who did the coding.

How to organize it so you can actually produce it under a deadline

Use a consistent file-naming and folder structure per study from day one, for example study name and date at the top level, then raw, coded, and memos as subfolders, so "find everything for the checkout study from March" is a folder lookup, not an investigation. Treat your synthesis tool (Dovetail, Condens, or a shared drive with real discipline) as the single source of truth, and avoid parallel copies living on someone's laptop or in a personal message thread that could contradict the official record. Keep the audit trail live, not retroactive: log coder and timestamp as you go, during the study, rather than trying to reconstruct who did what six months later when compliance actually asks.

When the request comes in, you're assembling an already-organized folder into a summary document, what was collected, how it was coded, who reviewed it, what changed and when, not searching your memory or your laptop under time pressure.

The honest limitation

If these habits weren't already running by the time compliance asks, you can't retroactively manufacture a clean audit trail. The best you can do going forward is start today, and be transparent about what does and doesn't exist for the older finding in question.

No step is optional if the recommendations need to survive a skeptical stakeholder later.

1. Prepare the transcripts. Verbatim text, personal information (PII, personally identifiable information) removed, tagged with metadata (participant type, date, which flow they were testing).
2. Choose tools for the workflow, not the workflow around the tools. A dedicated research repository (for example Dovetail or Condens) makes tagging and searching across sessions faster than a spreadsheet once you're past a handful of transcripts, but a well-organized spreadsheet works fine at a small scale; the process discipline below matters more than which tool holds it.
3. Build and pilot the codebook, following the same draft-pilot-reconcile cycle as any qualitative coding project: draft from the research questions, pilot on a small sample with two coders, and refine the definitions until they agree.

Worked example

Twelve transcripts about a project-management tool's task-assignment flow. Two coders pilot-code three of the twelve transcripts (25% of the set) for the code "assignment confusion," across 30 coded segments.

$p_o = (7+20)/30 = 0.90$. $p_e = (0.30 \times 0.267) + (0.70 \times 0.733) = 0.080 + 0.513 = 0.593$. $\kappa = (0.90-0.593)/(1-0.593) = 0.307/0.407 \approx 0.75$, comfortably above the usual 0.7 acceptance bar, so the team proceeds to code the remaining nine transcripts independently.

Once all twelve are coded, "assignment confusion" appears in 9 of 12 transcripts (75%, high frequency) and is tied to missed deadlines in the quotes (high severity), while the fix (requiring an assignee before a task can be created) is a small, single-form change. That combination lands it as a quick win. A second theme, "notification overload," appears in only 4 of 12 transcripts (33%) and is described as annoying rather than task-blocking, and the fix needs a new preferences panel (medium effort), so it's logged but prioritized behind the quick win.

Trade-offs and pitfalls

• A formal reliability check like this one is worth the overhead when the recommendations are going to leadership or into a roadmap decision; it's overkill for a same-day, low-stakes usability read.
• Don't let "impact" quietly become "what the loudest stakeholder in the room wants": tie it to the frequency and severity counts in the data, not to who's advocating for it.
• Effort estimates from a researcher alone are a guess; always confirm them with the engineering or design team before publishing the impact-versus-effort view, since a "quick win" mislabeled as small can undermine trust in the whole matrix.
