Retrospectives and Continuous Improvement Questions

Situation: After a disappointing sprint where we missed committed stories and had recurring bugs, I would run a focused retrospective to surface root causes and produce actionable improvements.

Task: My goal is to create a safe environment for candid feedback, identify 2–3 high-impact improvements, and convert them into measurable, owned actions for the next sprint.

Action:

1. Set the stage (5–10 min) — remind team of the goal: learn, not blame. Use a short safety check (fist-to-five) to ensure psychological safety.
2. Gather data (10–15 min) — use a timeline or “What went well / What didn’t / Confusions” board. Tools: Miro, Mural, or a physical whiteboard + sticky notes.
3. Generate insights (15 min) — use “5 Whys” or Fishbone for top themes; vote with dot-voting to prioritize issues.
4. Decide on actions (10–15 min) — convert top 1–3 problems into SMART experiments. For each action, define: owner, success metric, and deadline. Example actions: enforce definition-of-done, add a short pre-grooming session, or add CI gate to reduce regressions.
5. Close (5 min) — recap decisions, document in Jira/GitHub issues (label: retrospective-action), and add to sprint planning as backlog items.

Result: Track actions as tasks with acceptance criteria and measurable KPIs (e.g., reduce escaped defects by X%, improve sprint predictability by Y%). Review actions in the next retro: what worked, what didn’t, and iterate. This keeps continuous improvement concrete and accountable.
