Located in Southern York Region, The Adult Day Program is funded by the Central LHIN (Local Health Integration Network). This program helps to divert and delay admission to long-term care by providing therapeutic, social, recreational and Jewish programming to seniors with Alzheimer’s disease and other dementias, as well as those aging with developmental disabilities and dementia. Services are delivered by a diverse team of health care providers.
We are looking for: 1 full-time contract Program Assistant (6 months)
Hours of Work: Mon-Thurs 8:30am-4:30pm and Fri 8:30am-3:30pm
Reports to: Supervisor, Adult Day Program
Responsibilities
- Support the clients as they participate in programs activities
- Assist with meals/feed clients and supervise client dining.
- Escort clients on Adult Day Program Transportation.
- Assist Kitchen Assistant in plating and serving lunch to the clients
- Assist clients with activities of daily living, eating, toileting and bathing.
- Administer medications to clients under the supervision of the RN/Supervisor
- Maintain a clean and safe program environment.
- Attend staff meetings, case reviews and ongoing education.
- Always maintain client privacy including client files and information.
Risk, Health and Work Place Safety
- Identifying and reporting health and safety incidents and concerns in a timely manner to the appropriate supervisors and/or funders.
- Participating in health and safety processes and procedures
- Participating in maintaining a safe workplace environment by cultivating a positive safety culture and encouraging best practices to promote both staff and client safety and well-being
- Participating in all health and safety training initiatives on a regular basis.
- Taking proactive action against client incidents within your scope of practice.
- Developing a plan to identify, manage and/or minimize client safety risks or situations in adherence with risk management operations policies
- Assessing the severity of an adverse client safety/risk event and determining the best follow-up and developing an action plan following the event. Collaborating with funder (ex. HCCSS) and following any additional processes as required.
- Calling emergency services (911) when the client is at an immediate risk of harming themselves or others, or if there is a serious injury and/or imminent harm.
- Evaluating any potential hazards and identifying clients at risk for adverse health and safety events, taking preventative measures when necessary to minimize reoccurrence.
- Reporting all safety events impacting clients, caregivers and families in a timely and honest disclosure.
Qualifications
- A Personal Support Worker (PSW) Certificate from a curriculum approved by the Ministry of Colleges and Universities or Ministry of Education OR Health Care Aide OR Foreign-trained Registered Nurse, Registered Practical Nurse.
- A valid CPR, First Aid Certificate
- A Food Handler Training Certificate is an asset.
- Previous experience in a community setting or Adult Day Program or Long Term Care is preferred.
- Experience working with frail and/or cognitively impaired elderly population and their families.
- Demonstrated knowledge and experience working with individuals living with Alzheimer’s disease and Dementia.
- Experience in assisting in recreational programming for seniors.
- Experience working with clients in a Jewish environment is an asset.
- Must be able to work effectively as part of multi-disciplinary team or independently as required.
- Communicating in a clear and effective manner, demonstrating respect, empathy and understanding.
- Proficiency in a second language skills is an asset
- An effective team player.
